Allow scrolling and moving around freely, but scroll and move the document so that the insertion point always stays at the same vertical position.

If you know 3D gaming, this is much like it: the camera and player are in fact fixed but the world moves them when the player hits WASD.

Or like a real typewriter would work, in fact, with the paper scrolling beneath.
