Screen reader support for blind users

Would it be possible for you to add some screen reader support for blind users?
Right now the biggest problem is its not possible to see what items are selected.
also, it would be a help if you could add some hotkeys for some of the most used functions.
I am working around things by using search routing the physical mouse and clicking and hoping for the best.
sometimes I have to repeat things 4 or 5 times until it works.
