fix TOC links to headers #9

The icons for the headers are ignored by GitHub when creating the HTML Anchor so need to update Table of Contents (TOC) to ignore them too. This allows people to click in the TOC and be taken to relevant section when viewing the rendered README on GitHub. Also recommend turning on GitHub Pages for this repo and making the README available as a website on https://miserlou.github.io/dnd-tldr/
