Miners - Block template generation fixes

Fix block template generation to correctly support chains of dependent mempool transactions.

BIP100 Fix

This is a mandatory immediate update.

Per the BIP100 spec, allow maximum block size to be decreased (as well as increased) from the initial default of 32MB.
